Heart Attack

These are more complexed. This just gives you a direction... If you throw out these terms they will think you are professional!  Some people don't like playing a doctor or nurse as they have no idea what to do.


Chest pain- can do the following 
        -Put on cardiac Monitor to watch heart
        -Vitals
        -start IV and get blood work called Cardiac Enzymes
       - EKG-checks heart rhythm
       - Put on Oxygen to the patient
        -Give IV pain medications (morphine)
       - Can do Heart cath (use the surgery room)
             use IV dye to see if there is a clot
        -Can while in heart cath pretend to go in and remove clot or put in a stent
       - Do by pass surgery (requires hospital stay after)
         
     
CPR- Do only if they patient had no pulse or heartbeat. Unlike the movies you CANNOT use       shock paddles on a patient who is pulseless or no heartbeat (called a flat-line or Asystole)
          Just add -Oxygen to the patient per ambu bag, start IV and give Epi  IV to jump start heart.
     
   
Now if you are doing Chest pain heart attack you can use paddles if they patient has what is called AFIB (heart rate too fast) or if you want to do an unconscious patient who is having a major heart attack you can shock ventricular fibrillation (VF) as his is right before flat-line and bring your patient back!  Give IV med Amiodarone 
If they heart rate is too slow (bradycardia) give Atopine IV meds to make it go faster.

All of this requires overnight hospital stay and they get to go to the ICU!


Cardiac Enzymes- measures levels of enzymes and proteins that are linked with injury of heart muscle
CPK
CK
Troponin
Myoglobin
      Just say all of these are high
